If your Medicare card is lost or stolen you can ask for a replacement card by visiting a Medicaid Service Centre or sending your new address to the Department of Human Services, GPO Box 9822 in your capital city together with a signed written request that includes your Medicare card number .
To get a new card all you need to do is to call the welfare office. They'll send you a new one.
